Please bring the following that relate to your problem. This will help us fully evaluate you:

• All previous medical records
• All previous x-rays (include CAT scans & MRIs), reports, and actual images
• Previous splints used or a description of the splint
• Referral
• Insurance card and information
• Email, or send or bring completed health questionnaire
• Photo ID

We have onsite x-ray to help provide comprehensive care. Emergent CAT scans and MRI’s can often be done the same day. So please plan accordingly.
